Ratings Events - $8 each

(Twirlers must be entered in competition events in order to sign up for ratings. We are not offering
a "ratings only" option for this contest. For all ratings events, the twirlers will judged based on their
performance vs their skill level and will receive a Level 1, 2, or 3 Rating, with 1 being the best).

PAYMENT OPTIONS: Once you submit this form it will be sent to us to start your twirler's registration.
You will also be redirected to PayPal for payment. If you prefer to use PayPal or a credit card, please continue on with this option.

If you would prefer to use a different form of payment, you can just close the PayPal screen. Payment will also be accepted
via Venmo (@twirlatx), CashApp ($TwirlATX), or check mailed to Twirl ATX, 1808 Saline Creek Dr., Cedar Park, TX 78613.

Registration will not be complete until payment is received. If your payment is not
received by the registration deadline, your registration will not be accepted.

Payment Options*

I will be paying with PayPal/Credit Card via the Pay screen after I hit submit. I understand that there is a $9 fee for using this method of payment.

I will be paying with Venmo/CashApp/Check. I understand that when I submit the entry form it will still take me to the PayPal page, and that I should just close the page and send my payment through my alternate method.

NO REFUND POLICY: There are NO REFUNDS for any reason, including change of personal plans/availability, change of venue, change of date,
change of judges, change of format, change of schedule, or change of safety requirements. The only exceptions where a refund could be given is:
(1) Contest Cancellation (2) Proof of a positive COVID test of a twirler or a family member that lives in the same household as the twirler within 5 days of the contest.

Participant Liability Release Form

This form must be signed by the contestant or a parent/legal guardian of each contestant under the age of 18 and received by the
contest directors before your entry will be processed. This includes all team members.

I (we) represent that I (we) are the competitor or the parent/legal guardian(s) of the minor (competitor) under the age of 18 years and, in order to
have you accept their competition entry, have accepted this document on behalf of myself or said minor and ourselves individually and jointly,
and agree to be bound by all terms and conditions in this liability release, in the same manner if we were the (competitor) twirler. Each of us
hereby releases the State Directors and/or TwirlATX LLC , and the High School, its officers, agents and all employees from any
and all liabilities. With the acceptance of this form, the undersigned agree to indemnify and hold harmless the above mentioned from any
liability in connection with this event.
